HEALTH COUNCIL OF NORTHWEST OHIO
The Health Council of Northwest Ohio (HCNO) serves an 18-county region with a unified goal: to improve health outcomes by addressing the social factors that shape well-being. HCNO uses a data-informed model to identify gaps and deliver targeted solutions through its nationally certified Northwest Ohio Pathways HUB. This evidence-based approach coordinates care and support for individuals facing complex challenges, using a network of trusted community partners.

GAPQUEST: GAP YEAR EXPERIENCE
GapQuest is a bold and innovative response to the urgent needs of today’s youth. Designed for recent high school graduates aged 18–20, GapQuest is an immersive, experiential program that bridges the critical transition from adolescence to adulthood.
Participants gain independence, engage in paid co-op work experiences, and take part in weekly personal and professional development challenges. Through coaching, mentoring, and hands-on experiences, young adults build the confidence, competence, and clarity they need to thrive.
